TAG Active (Risk Acknowledgment Disclaimer)

THIS IS AN IMPORTANT DOCUMENT AND MUST BE READ & UNDERSTOOD BEFORE SIGNING

TAG Active is a physically challenging assault course style arena and by participating you understand that the activity is high risk and failure to follow the safety rules could result in serious injury.

TAG Active | Under 18 age information

• Minimum age, 8 years of age and over 1.2m in height.

• Anyone under 18 years of age wishing to participate in TAG Active must have signed parental / guardian consent form.

By completing and signing this agreement you are consenting to the terms of this acknowledgement and disclaimer and you are agreeing to your designated minor’s participation in the events within TAG Active Ipswich. This document is valid for 12 months from completion of this form.

TAG Active | Prerequisites for entry / participation.

• I am the parent/guardian of the child/ren listed below who is/are under 18 years of age. I consent to allow that/those child/ren to participate in TAG Active arena within 12 months of the date of completion of this form.

• I declare that if I am not the parent or guardian of the child/ren I have the authority from the child/ren’s parent or guardian to sign this risk acknowledgement disclaimer form.

• I acknowledge the risk on their behalf.

• I confirm that I am responsible for the child/ren in my care and undertake to ensure that he/she/they undertake TAG Active in accordance with the ‘TAG Active Arena Rules’ and ‘TAG Active Dress Code’ made available to me prior to participation and at / within the arena in addition to the advice he/she/they are given in the pre-participation safety briefing, together with any oral instructions or advice given to he/she/them prior to or during the session.

• I acknowledge that I am responsible for the safety & supervision of the child/ren named below (and the safety of our possessions) and that TAG Active Ipswich do not provide supervision of children whilst using the premises or if left unattended on the premises.

• I certify that to the best of my knowledge the child/ren is/are in good physical condition and do not have any medical condition which might have the effect of making it more likely that he/she/they be involved in an incident which could result in injury to him/her/themselves or others (if in any doubt, please check with the child/ren’s parent or guardian).

• I certify that the child/ren have no pre-existing medical conditions which, could through participation in TAG Active, result in injury to them or damage their health.

• I certify that to the best of my knowledge the child/ren is/are not pregnant.

• I acknowledge that the maximum permitted weight for participation in Ninja TAG Active is 20.5 stone (130kg) and confirm that the child/ren do/does not exceed this weight.

• I certify that to the best of my knowledge the child/ren do not knowingly suffer from vertigo or claustrophobia.

• I certify that to the best of my knowledge the child/ren is/are not at the time of participation under the influence of alcohol; and/or any illicit drugs; and/or prescribed medications that might impair my ability to undertake the activity safely.

• I agree that TAG Active will not have any liability to the child/ren for any loss or damage of clothing or other personal items resulting from participation in Ninja TAG Active.

• I agree that in order to supervise the child/ren I must be a registered user in the TAG Active game session.

TAG Active | Dress Code

• Shoes – Wear sensible sport shoes. (Open toes shoes, croc type shoes, espadrilles or open back shoes must not be worn)

• Clothing – Wear suitable full body clothing, i.e long sleeve top and full length bottoms.

• Secure all drawstrings prior to participation. Do not wear items of clothing with sharp items on them. Do not wear torn clothing. Remember to wear clothes you don’t mind getting dirty or even slightly damaged.

• Jewellery – Remove all jewellery. Please do not bring these to the session. Lockers are available.

• Hair – Secure tie back loose hair prior to participation.

• Loose Items – You must not have anything in your pockets or attached to you that may fall. Please use the lockers provided. This includes all electronic devices.

• Medication – Bring all medication you may need throughout your activity.

• Food / Drink – Do not take food or drink into the TAG Active arena.
